Ministry Partner Application Process

 This three-step application process is to be completed by all potential ministry partners involving the supervision of minor children, youth or vulnerable adults. The purpose of these forms is to assist in the creation of a safe environment for the programs of Woodmen Valley Chapel.

Please complete all three steps below. You will need to download, complete, sign and mail the Vounteer Safety Application and the CBI Form. The last form may be completed online. One of our Coordinators will be contacting you once we have received these three forms.

1. Ministry Partner Application

Please CLICK HERE to download, complete, sign and mail a KidsCove Code of Conduct and information sheet.

Please address the envelope to the Ministry Leader who requested your information and leave it at the Children's Greeter Desk or mail it to: Woodmen Valley Chapel, 290 E. Woodmen Road, Colorado Springs, CO 80919.

2. Criminal Background Investigation (CBI)

CLICK HERE to download the CBI form. Please address the envelope to the Ministry Leader who requested your information and leave it at the Children's Greeter Desk or mail it to: Woodmen Valley Chapel, 290 E. Woodmen Road, Colorado Springs, CO 80919.

3. Sexual Misconduct Policy (SMP)

CLICK HERE to read policy. Then please return to this page to complete and submit the acknowledgement that you have received and read the policy. 

I hereby acknowledge that I received a copy of the Woodmen Valley Chapel Sexual Misconduct Policy. I have read the policy and I understand its requirements. As a condition of my employment or my volunteer engagement with Woodmen Valley Chapel, I agree to comply with the terms of the policy.
